**Job Function**Case Management**Reference Number**MWS/221**Advertised**44 minutes ago**Experience Required**2 to 5 years**Minimum Qualification**Post-Graduate Diploma**Job description****Job purpose**- The Medical Social Worker works closely with Agency for Integrated Care (AIC) on referrals from hospitals and government-linked agencies and other relevant stakeholders to support aging in place for patients and their families in the community. The incumbent is responsible to support a coordinated approach with multi-disciplinary stakeholders to regularly review bio-psycho-social-spiritual aspects to ensure holistic care.**Duties and responsibilities**- To provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large- Participate in multi-disciplinary team meetings in developing individualised care plans and facilitate family conferences for patients/clients and family members together with members of the multi-disciplinary team for facilitation of effective communication- Assess caregiver stress and burden and provide supportive platforms and initiatives for caregivers- Assessment of risk and to intervene in a timely manner- Information and referral services for patients and family members as deemed necessary for patient/client's optimal functioning in the community with relevant resources- Undertake projects and any other duties as assigned by Reporting Officer or Head of Centre/Centre Manager**Job Requirements****Qualifications**- Bachelor/Post Graduate Diploma in Social Work- Preferably a registered Social Worker with the Singapore Association of Social Workers (SASW)**Working conditions**- Any deployment as designated within MWS Integrated Resource Group Social Work - Community Eldercare Services- Office hours. Monday to Fridays. May be required to extend beyond office hours or on weekends if required.- Physical requirements- Physically fit and able to meet the requirements of the role**Core competencies**- Good team player with positive attitude, caring and compassionate individual- Good interpersonal skills, able to communicate people at all levels such as patient's families, government agencies and other professionals- A quality service mind-set- Preferably with at least 2 years of relevant working experience in a healthcare or eldercare setting**Technical Competencies**- Basic computer knowledge, familiar with MS Office tools and Internet- Familiarity with dialects will be an advantage- Advance Care Planning facilitator would be an advantage- Inclination towards palliative care and/or community case management will be an advantage- Ability to speak and write effectively in English.
